Replacement of Nationality Certificate

Please follow the guidelines below in submitting the necessary documents for the replacement or transfer of your nationality certificate. This will alleviate delays in the processing of your certificate.

  • White-out is not accepted
  • Minimum scratches on forms
  • Supporting documents must be in good condition and legible
  • Applicant’s signature on documents must be consistent with signature on passport/ID
  • Original documents must be presented along with copies
  • Declaration Form can be purchased at your nearest stationery store
  • Documents in language other than English must be translated by a licensed authority

Citizenship by Registration

  1. Police Report
  2. Copy of certificate or certificate number (If available)
  3. Declaration requesting replacement, also stating how original certificate was lost
  4. Copy of passport biographic page or official form of identification
  5. Copy of birth certificate & translation is required for certificates that do not include date of birth and/or dependent on records present on file
  6. 3 photographs (must be 2×2, white background, no sleeveless, taken within the last six months, full face view directly facing the camera)

  1. Original Belize Nationality Certificate
  2. Declaration requesting replacement of certificate and reasons why
  3. Copy of passport biographic page or official form of identification
  4. Copy of birth certificate & translation is required for certificates that do not include date of birth and/or dependent on records on file
  5. 3 photographs (must be 2×2, white background, no sleeveless, taken within the last six months, full face view directly facing the camera)

  1. Declaration requesting the issuance of a separate certificate
  2. Copy of parent’s Nationality Certificate, showing name to be transferred to new certificate
  3. Copy of passport biographic page or official form of identification
  4. Copy of birth certificate and translation is required for certificates that do not include date of birth and/or dependent on records on file
  5. 3 photographs (must be 2×2, white background, no sleeveless, taken within the last six months, full face view directly facing the camera)
  6. Renunciation for persons born in Guatemala or India

Citizenship by Descent

  1. Police Report
  2. Copy of certificate or certificate number (if available)
  3. Declaration requesting replacement of certificate
  4. Copy of passport biographic page or official form of identification
  5. Copy of Birth Certificate and translation is required for certificates that do not include date of birth and parent’s name
  6. 3 Photographs (must be 2×2, white background, no sleeveless, taken within the last six months, full face view directly facing the camera)

  1. Original Belize Nationality Certificate
  2. Declaration requesting replacement of damaged certificate
  3. Copy of passport biographic page or official form of identification
  4. Copy of Birth Certificate and translation is required for certificates that do not include date of birth and parent’s name
  5. 3 Photographs (must be 2×2, white background, no sleeveless, taken within the last six months, full face view directly facing the camera)

  1. Original Memo Letter or Police Report if lost
  2. Declaration requesting replacement of memo letter certificate
  3. Copies of all previous Belizean Passports
  4. Copy of Birth Certificate and Translation
  5. Copy of Parent’s Belizean passport or birth certificate and identification or death certificate
  6. 3 Photographs (must be 2×2, white background, no sleeveless, taken within the last six months, full face view directly facing the camera)
