Lost, Stolen or Mutilated Passports

Applying in Belize

For the applicant whose previous passport is now unavailable for submission (be it lost, stolen or mutilated), the applicant shall submit

  1. If lost, stolen or mutilated in Belize, the statutory declaration in Form 6 detailing the incident leading to the unavailability of the passport that is certified by a Justice of the Peace, Commissioner of Affidavit or Notary Public
  2. A report from the Police indicating that the incident leading to the unavailability has been recorded

Applying Overseas

For the applicant whose previous passport is now unavailable for submission (be it lost, stolen or mutilated), the applicant shall submit

  1. If lost, stolen or mutilated outside Belize, the statutory declaration in Form 6 that is notarized by a Notary Public detailing he incident leading to the unavailability of the passport.
